Cortex is a game that trains clinical reasoning. You work up patient cases — order tests, reach a diagnosis, choose treatment — and earn XP, coins and trophies.
Cortex helps you learn medicine through realistic clinical cases rather than memorizing isolated facts. Each case is designed to simulate real clinical decision-making, guiding you through history taking, examination findings, investigations, differential diagnoses, management, and key learning points. Whether you’re preparing for exams, clinical placements, or everyday practice, Cortex helps you build knowledge in a structured, practical, and engaging way.
Yes. Cortex is designed to strengthen clinical reasoning by encouraging you to think through cases step by step instead of simply reading the diagnosis. As you work through diverse clinical scenarios, you’ll practice recognizing patterns, generating differential diagnoses, selecting appropriate investigations, and making management decisions. While no app can replace real patient experience or clinical supervision, regular case-based practice can help you develop a more systematic and confident approach to clinical problem-solving.
Each case is worth up to 100 XP. You earn the full 100 only for a flawless workup; your score reflects how accurate your tests, diagnosis and treatment were.
Energy refills gradually over time, up to a full bar. Your daily case is always free to play. Practice, random and past cases each cost 16.5 energy — or just 8.5 with Cortex Pro, so you can play about twice as many. Emergency cases don't cost energy, but they're a Pro perk.
Play at least one case a day to keep your streak alive. Hitting milestone streaks rewards bonus coins, and a Streak Freeze from the shop can protect you on a day you miss.
Coins are earned by solving cases and hitting streak milestones. Spend them in the shop — for example on Streak Freezes.
Pro unlocks the full experience — including AI case explanations, cheaper energy (about twice as many cases) and Pro-only emergency cases. It's an auto-renewing subscription billed through your Apple ID on iOS or Google Play on Android, and you can cancel anytime in your device's subscription settings.
Yes — with Pro, “Ask AI about this case” gives you an AI-generated explanation of the case you just completed, so you can dig into the reasoning behind the diagnosis and management. It's for learning only and is never medical advice.
Occasionally a time-sensitive “emergency” case appears for a few hours. Solve it while it's live for a special challenge. Emergency cases are a Cortex Pro perk.
Once you've solved today's daily case, you can replay previous daily cases from the calendar on the Home screen. Past cases cost energy like a random case.
